At Lotus Rise, we understand that transitioning into adulthood can be a challenging time. That’s why we provide a smooth, supportive pathway for individuals moving from our Alternative Provision (AP) into our Supported Living services.

Our integrated approach ensures continuity of care, familiarity, and personalised support to help young people thrive as they take the next steps toward independence.

The Transition Pathway

Step 1: Early Planning

We start the transition process early, working with families, schools, and professionals to create a plan that supports the young person’s individual needs and goals.

Step 2: Familiarisation Visits

Individuals are encouraged to visit our Supported Living services to meet the team, explore the environment, and build confidence before transitioning.

Step 3: Tailored Support Plans

Our person-centred support plans are created collaboratively, ensuring every aspect of the individual’s needs – from daily living skills to mental wellbeing – is met.

Step 4: Building Skills for Independence

Through our Alternate Provision services, young people develop the following skills, which form a strong foundation for success in Supported Living.

  • Life skills such as budgeting, cooking, and self-care.
  • Behaviour management strategies.
  • Social and communication skills to engage with their communities.
  • Adult educational qualifications such as Functional Skills

Step 5: Ongoing Support

Once in Supported Living, individuals continue to receive tailored care, empowering them to live meaningful and independent lives.
